WHAT IS LOW BACK PAIN? (1) Low back pain (LBP) is pain or discomfort felt between the bottom of the ribs and the top of the buttocks. Understanding Low Back Pain P.5 Low back pain can be divided into two common types: 1 - World Health Organization. (2021, June 17). Low back pain. https://www.who.int/news-room/fact-sheets/detail/low-back-pain 2 - Wernli, K., O’Sullivan, P., Smith, A., Campbell, A., & Kent, P. (2020). Movement, posture and low back pain. [Epub 14 February 2017]. doi:https://doi.org/10.7326/M16-2367 THE ORIGIN OF LBP ? (2;3) Mechanical low back pain (99% of cases) comes from the spine’s structures, like muscles, ligaments, discs, or joints; it usually gets worse when you move, lift, bend, or sit for a long time. Pathologic low back pain (1% of cases) is caused by a specific medical condition, such as infection, fracture, tumor, or nerve compression. Different types of non-specific back pain (4) It is not possible to diagnose the exact cause of this back pain in most people. Understanding Low Back Pain The main manifestations of low back pain • Benign low back pain (1) is a broad term for common lower back pain that is usually not linked to a serious medical problem. It can be caused by muscle strain, poor posture, or everyday movements. (1) Margetis, K., Singh, C., Casiano, V. E., & Varacallo, M. A. (2025, 13 décembre). Low Back Pain : Evaluation and Management. Understanding Low Back Pain • Sciatica (1) Sciatica is pain that starts in the lower back and can travel through the buttock and down the leg. It happens when a nerve in the lower spine is irritated or compressed, often because of a herniated disc or narrowing of the spine. It may also cause numbness, tingling, or weakness in the leg. Managing low back pain usually involves several healthcare professionals working together to help you feel better. YOUR CARE TEAM General Practitioner (GP): Your main doctor who helps you understand your pain and guides your treatment. Radiologist: Provides imaging studies when clinically indicated to assess causes of LBP. However, imaging is not always required Rheumatologist: Involvement if LBP is suspected to be inflammatory (e.g., axial spondylarthritis*) , with specific tests and long-term management Physiotherapist: Guides exercise therapy, manual therapy, and education, which are first-line treatments for LBP. Dietitian/Nutritionist offers advice on weight management and healthy dietary habits, which may influence LBP severity Occupational therapist: Helps adapt work and home environments to reduce LBP triggers and promote function. Orthopaedic surgeons: Consulted for severe structural problems (e.g.: a herniated disc causing leg weakness or numbness). Surgery remains rare and only considered after conservative treatments fail. Psychologist/Behavioral Therapist: Addresses the psychological factors (fear-avoidance, catastrophizing) that can maintain or worsen chronic LBP. (1) Maher et al., Lancet, 2017: "Low back pain series" (référence clé sur LBP management, recommandant peu d'imagerie sauf exception). WHAT CAN YOU ASK TO YOUR GP ? If you have recently been diagnosed with LBP, it can be overwhelming. However, education is crucial, and we encourage you to ask your doctor any questions you may have. If you need assistance in formulating questions, here are some suggestions: • What are the common causes of low back pain, and how do they differ from other types of back issues? • How might my low back pain progress or evolve over time? • Are there any potential complications or associated problems with low back pain that I should be aware of? • What are the available treatment options for managing low back pain? • Is it possible to prevent or minimize the progression of low back pain through treatment or lifestyle changes? • Which specialists or healthcare professionals should I consult for my low back pain care? • Is treatment for low back pain covered under my health insurance plan, and what are the associated costs? • Can I continue participating in sports or physical activities with low back pain, and are there any modifications needed? • What should I do if I experience severe or unmanageable low back pain? • Would consulting a psychologist or counsellor help me cope with the impact of low back pain on my daily life? The Effects of Low Back Pain on Youth P.23 When does it start? Back pain can begin as early as age 14. It’s often mild and temporary, but for some teens, it can affect daily life (school, sports, sleep). Why does it happen? - Too much training (e.g. gymnastics, dance) - Not enough movement (too much sitting) - Being overweight - Stress or anxiety What are the symptoms? - Back pain - Neck pain or headaches - Tiredness when sitting or standing How is it treated? - Learn about your pain - Stay active (don’t stop moving!) - Build good habits (posture, stretching, sleep) - Talk to someone if pain makes you anxious or sad What are the risks if it lasts? - Around 1 in 5 teens may have chronic back pain at age 17 - It can lead to missed school, less activity, or long-term care (1) O’Sullivan, P., Smith, A., Beales, D., & Straker, L. Living with Low Back Pain P.25 Low back pain affects about 4 out of 5 people during their lifetime and is on of the leading causes of disability in the world according to the World Health Organization (WHO). It affects both young and older workers, with significant consequences on personal and professional life. (47) At work : - Many workers with low back pain report difficulty maintaining their jobs due to pain. A report by the French Health Insurance Agency shows that in 2017, 30% of sick leave lasting more than 6 months was due to low back pain (ameli.fr). (1;2) - Keeping your job is easier when work conditions and the workspace are adapted to individual needs. (1;2) At home: - Pain can impact sleep, sexuality, leisure or parenthood. - A study published in Pain Practice (Volinn, 2018) points out that chronic lower back pain is associated with reduced social interactions and an increased risk of anxiety or depression. WHAT ARE THE POSSIBLE COMPLICATIONS ASSOCIATED WITH LBP ? - Chronic pain: Pain lasting more than 3 months is considered chronic. It affects 1 in 10 people but accounts for 80% of healthcare costs related to back pain. (1) - Reduced mobility: Chronic pain can limit movement, balance, and daily activities. (2) - Nerve pain: Conditions like sciatica or herniated discs can cause radiating pain, numbness, or tingling. (3) - Mental health impact: Limited movement can cause psychological distress, which worsens pain in return. Your health beliefs and coping strategies also affect your distress and pain levels. For example, anxiety, pessimism, and catastrophic thinking can make pain much worse by changing your brain. Psychological therapies effectively treat back pain-related anxiety. Mindfulness (MBSR) (8 weeks) has the strongest evidence, reducing pain and activating brain relaxation. Cognitive Behavioral Therapy (CBT) changes pain perception and catastrophic thinking. Hypnosis and progressive muscle relaxation also help. These modify brain biology to break the pain-anxiety cycle. (4) - Social isolation and loss of independence: especially in older adults. (5) => Early care and healthy habits can help prevent these complications. (1) Fréquence Médicale. (n.d.). The impact of continuous use of lumbosacral orthoses on trunk motor performance : a systematic review with meta-analysis. The Spine Journal …LUMBAR SUPPORT DOES NOT INDUCE MUSCLE LOSS (1;2;3) Clinical studies demonstrate that… No evidence in the literature suggesting that lumbar supports lead to muscle atrophy : No deleterious effect on muscle morphology The thickness of deep trunk muscles (obliques, transverse abdominals, and multifidus) in patients with chronic LBP, measured by ultrasound in different positions, is not affected by wearing a lumbosacral brace for 4 weeks in combination with routine physical therapy1. No deleterious effect on muscle activity The muscle activity of superficial back muscles, measured by electromyography*, may be reduced when wearing lumbosacral brace2, it is a protective mechanism and not a proof of dependence. It is associated with lower need for co-contraction due to the additional passive stiffening provided by the brace. The magnitude of this reduction is not sufficient to induce muscle atrophy. No deleterious effect on muscle performance Wearing a lumbosacral brace for one to six months do not have a negative effect on the strength, endurance, and fatigability of the trunk flexors and extensors3. Lumbosacral braces do not produce complete immobilization, likely preventing muscle atrophy. *Electromyography (EMG) : is a test that checks how well your nerves and muscles are working. It measures the electrical activity in the muscles and can help find nerve or muscle problems. NB1 MV2
