This guide is written for people living with cognitive changes, their carers, families, and professionals. It is designed as a companion to be used over time, not read cover to cover. The author, Nicolette McDermott-Ketchum, is a physician assistant and certified dementia practitioner who facilitates the Hummingbird Memory Cafe.

The guide opens with a dedication and instructions on how to use the material. Each section follows a consistent structure: Why This Matters, Did You Know?, Understanding This Dimension, Pause and Reflect, My Notes, and One Small Next Step. Selected resources for learning and support are included throughout.

The first dimension covered is Diagnosis. It explains why a diagnosis matters, notes that many causes of cognitive changes are treatable, and lists the emotions a diagnosis may bring. A diagram shows the diagnostic pathway from person and family to primary care, specialists, and testing.

The table of contents lists the full set of dimensions: Diagnosis, Care Coordination, Education, Advocacy, Physical Needs, Emotional and Social Needs, Creativity, Spiritual Needs, Safety, Transportation, and Legal and Financial Services. The guide ends with a resource section, information about the Hummingbird Memory Cafe, and an author bio.
