This certifies that the bearer of this Activity Card is a bona fide member of the Union of Filipino Tourism Students – Davao del Norte State College Chapter (UFTS–DNSC). This Activity Card shall serve as the official record of attendance for every institutional and organizational event, as required by the organization. The bearer must present this Activity Card for proper validation of time in and time out during each activity or event. Any loss or damage to this Activity Card must be reported immediately to the UFTS–DNSC Secretary for replacement and documentation.
