HMO Advisory: 2026 Benefit Enhancements and Dependent Enrollment 1. Expanded Animal Scratch Coverage Effective this renewal, animal scratch coverage is now available to all enrolled members, including eligible dependents. 2. Increased Maximum Benefit Limit (MBL) for Staff Employees Good day, Team! We are pleased to announce the latest enhancements to our HMO benefits and provide important reminders regarding dependent enrollment. HMO Benefit Enhancements Please note that the corresponding premium will be adjusted based on the approved rates for the enhanced MBL. 3.Point of Service (POS) Reimbursement for Non-Accredited Physicians Principal members and enrolled dependents may consult non- accredited physicians and request reimbursement after paying the consultation fee. To qualify, the member must have an active HMO plan, the consultation must be covered, and the required documents (such as the official receipt, medical certificate, prescription, and claim form, if needed) must be submitted. All claims are subject to HMO review and approval.

Note: Employees may enroll up to five (5) eligible dependents; however, only two (2) dependents are sponsored by the company. The premium for any additional dependents shall be shouldered by the employee.
