This document is a safety guide for the EPCIC of the Kasawari Gas Development Project. It is intended for workers and staff involved in the project to understand and avoid line of fire hazards.
The guide defines 'Line of Fire' as being in harm's way and categorizes hazards into three types: struck-by hazards, caught-in or between hazards, and energy released hazards.
Examples of line of fire situations include staying in the path of a moving vehicle like a forklift or mobile crane, placing hands or body in rotating equipment, working underneath a lifted load, using defective electrical equipment that could cause electric shock, working at height without a body harness, working without supervision, working near a hydrotest area, and improper cable arrangements that may cause slips, trips and falls.
The guide states that the best method to avoid line of fire is to eliminate the hazard itself. It also includes an HSE alert about a slip and fall injury in a pantry on 15 September 2021 due to a wet floor, where the injured person experienced left shoulder pain and was granted three days of medical leave.
