This is an undated postpartum care planner of 28 pages, made for new mothers and the people supporting them through the fourth trimester. It mixes pages for the parent's own recovery and rest with logs for baby feeding, pumping and diapers, and it can be reprinted for personal use as often as needed.

The opening section, Begin Gently, explains how to use the book: start with the essentials, add a log or support page only if it helps, and treat blank spaces as welcome. It also states that the planner is an organizational and reflection tool, not medical advice, and that a completed checklist is not proof that parent or baby are well.

The first planning pages are Just the Essentials, a short daily page for date, current feeling, one priority, one need, who can help and what can wait, and the Daily Care Planner, which adds energy level, food and water, rest, baby care, appointments and help to ask for. A filled-in example day shows a light plan with low energy, a lunch request and laundry handed to a partner.

The Care for You chapter covers recovery notes for recording the care team's instructions and medicines, a weekly care checklist with a Monday to Sunday key, a self-care menu sorted by available energy, comfort and connection, and pages for mood and feelings, rest and sleep, water and nourishment, and meals with a grocery list.

Care for Baby holds the feeding log, pumping log, diaper log and general baby care notes. The closing section, Support and Reflection, provides space for appointments and questions, a care team list, a support plan, visitors and boundaries, caregiver handover, weekly reflection, small wins and intentions, and notes to keep. A final page covers printing and personal use.
