PSYCHIATRIC EVALUATION REPORT’ PATIENT INFORMATION: Name: Joe Espinosa Gender: Male Date of Birth: June 03, 2000 Age: 26 Report Release Date: August 29, 2026 Evaluation Date: August 29, 2026 Occupation: Police Officer Evaluator: Dr. Jade Lin, MD, PhD, FRCS, FACS REFERRAL INDICATION: Mr. Espinosa was referred for psychiatric fitness-for-duty evaluation following an accidental firearm discharge approximately three weeks ago, wherein he intended to deploy his taser but mistakenly discharged his service firearm. RELEVANT OCCUPATIONAL HISTORY: The patient reported that during the incident, he had previously drawn his firearm and failed to holster it. He was reportedly unaware that he was still holding the firearm when he intended to use his taser. Due to the incident occurring approximately three weeks ago, certain details regarding the exact sequence of events remain unclear to the patient. A previous work-related lapse was also documented involving the accidental removal of a suspect's handcuffs. At the time, the patient reported working approximately 13 hours per day. CLINICAL OBSERVATIONS: During the evaluation, the patient presented as alert, calm, coherent, and appropriately communicative. He was able to explain himself clearly, with no observable signs of significant anxiety or emotional instability.
Some defensiveness was observed when discussing his previous psychological evaluation and the shooting incident. However, the patient demonstrated significant remorse, acknowledged the seriousness of the incident, and showed appropriate understanding of its consequences. MENTAL HEALTH ASSESSMENT: General Behavior: Calm and cooperative Speech: Clear, coherent, and appropriate Thought Process: Logical and organized Mood and Affect: Stable and appropriate Anxiety: No significant manifestations observed Insight: Adequate Judgment: Intact during evaluation CLINICAL IMPRESSION: No current evidence of acute psychiatric instability, significant cognitive disturbance, or behavioral impairment was observed during the evaluation. The patient demonstrated adequate emotional regulation, communication, insight, and judgment at the time of assessment. CONCLUSION AND RECOMMENDATION: Based on the findings of the present psychiatric evaluation, Mr. Joe Espinosa is considered psychologically stable and fit to resume full occupational duties as a Police Officer. At the time of assessment, no significant psychiatric symptoms, emotional instability, or behavioral impairment were observed that would warrant restriction from duty. The patient demonstrated appropriate communication, emotional regulation, insight, and understanding of the seriousness of the previous incident. It is therefore recommended that the patient be cleared as FIT TO WORK and may return to full duty without psychiatric restrictions. Continued adherence to appropriate rest periods, departmental safety protocols, and proper firearm and taser handling procedures is advised
