NAVAL ST., HULONG-DUHAT, MALABON CITY PARENTAL CONSENT AND AUTHORIZATION Extended Laboratory Research Preparation for the Division Science and Technology Fair 2026 I,________________________, parent/guardian of __________________________, a Grade 10 STE student of Malabon National High School, hereby give my consent for my child to voluntarily stay in the school Science Laboratory after the regular 2:20 PM class dismissal until 5:00 PM, from August 14 to August 26, 2026, for research preparation and related activities in connection with the upcoming Division Science and Technology Fair (DSTF) 2026. I understand that the extended preparation period is intended to provide student researchers additional time to finalize their research manuscripts, required enclosures, data, documentation, and other research requirements before the DSTF submission deadline. I further understand and agree that: 1. My child's participation in the extended laboratory preparation is purely voluntary and will not be required if I do not give my consent. 2. My child will remain in the school premises only until 5:00 PM and will leave the school after the scheduled preparation period. 3. The extended laboratory sessions will be conducted under the supervision of the Science Teacher/Research Adviser. 4. My child is expected to follow all school rules, laboratory safety protocols, and proper procedures in handling laboratory materials and equipment. 5. I have been informed of the schedule and purpose of the extended laboratory preparation and have been given the opportunity to ask questions regarding my child's participation. 6. I understand that I am responsible for arranging my child's safe transportation home after the 5:00 PM dismissal, unless otherwise agreed upon with the school. 7. I may withdraw my consent at any time by informing the Science Teacher/Research Adviser or the school administration. By signing below, I confirm that I have read and understood the information above and voluntarily authorize my child to participate in the extended laboratory research preparation from August 14 to August 26, 2026, until 5:00 PM.
