Group Short Term Disability Insurance First Day Hospital Benefit The first day hospital benefit is designed to pay disability benefits from the first day you are admitted to a hospital. This benefit can help you pay for your medical bills and other expenses, so you can focus on your care and recovery. How it works If you select a plan with an elimination period of 30 days or less, this benefit waives that elimination period from the first day of hospital confinement for a total disability due to a covered accident or covered sickness. This means you can begin receiving disability benefits from the first day you are confined to a hospital. Disability benefits will continue even after you are discharged, as long as you continue to have a covered disability. Confinement means you are admitted to a hospital and confined as a resident inpatient (including intensive care) on the advice of a physician. Group Term Life Insurance Voluntary coverage Our group term life insurance can help increase financial security for your family if something were to happen to you. You can also apply for coverage for your spouse and eligible dependent children without health questions.1 How much group term coverage do I need? Additional benefits and services Built-in accelerated death benefit provides an advance of up to 75% of the death benefit, to a maximum of $150,000, if diagnosed with a terminal illness.3 Health Advocate Employee Assistance Program (EAP) provides 24-hour personal support and referral service, including a medical bill saver service. Face-to- face sessions and video counseling with mental health professionals are available.4 • Online: ColonialLife.com/EAP • Telephone: 1-888-645-1772 Life planning services offer financial and legal counseling services, as well as grief support and referral for up to 12 months after a claim.4 Get the most out of your coverage Portability: You may be able to continue your coverage if you change jobs or retire. Conversion: After the group term period ends, you may be eligible to convert to a whole life policy without health questions. Waiver of premium: Your premium payments may be eligible for waiver if you become disabled. To learn more, talk with your Colonial Life benefits counselor. 1 Spouse and dependent coverage will not be effective if they are currently totally disabled. Being totally disabled means the inability to perform two or more activities of daily living, being confined to a hospital or similar institution, or being unable to attend school outside the home (for a dependent child age 5 up to age 26). In ID, NH and TX, the definition of total disability does not include Activities of Daily Living (ADL) requirements. The ability to work does not determine disability. You can pay premiums on insurance for your dependents with no health questions asked. Coverage isn’t effective until the earlier of the date they are no longer totally disabled or two years after the date that coverage would have otherwise become effective for the spouse or dependent child. This provision does not apply to newborn children born while dependent insurance is in effect. 2 The maximum benefit is 50% of your benefit in NE. 3 Terminal illness means an injury or sickness that results in the covered person having a life expectancy of 12 months or less and from which there is no reasonable prospect of recovery. A life expectancy of 24 months or less in IL, KS, MA, TX and WA. Accelerated death benefit payments will reduce the amount the policy pays upon the recipient’s death, may adversely affect the recipient’s eligibility for Medicaid or other government benefits or entitlements, and may be taxable. Recipients should consult their tax attorney or advisor before utilizing accelerated benefit payments. 4 The Employee Assistance Program and Life Planning Services, provided by Health Advocate, are available with Colonial Life & Accident Insurance Company Group Term Life offering. BENEFIT AGE REDUCTION SCHEDULE When a covered person reaches age 70, but not 75, the amount of insurance will be: • 65% of the amount of insurance prior to age 70; or • 65% of the amount of insurance applied for on or after age 70 but before age 75. When a covered person reaches age 75 or more, the amount of insurance will be: • 50% of the amount of insurance prior to the first reduction; or • 50% of the amount of insurance the employee applied for on or after age 75. Once the benefit reduction schedule begins, there will be no further increases in insurance for a covered person. If the proposed insured is age 70, but not age 75 at the time of enrollment, the amount of insurance applied for will be reduced by 65%. If the proposed insured is age 75 or older at the time of enrollment, the amount applied for will be reduced by 50%. How accident insurance works With accident coverage, you have stronger protection so you can focus on your health and recovery instead of worrying about paying your medical expenses. Here’s how it works. • A set amount is payable based on the injury and the treatment needed, for simple and complex injuries. • Benefits are payable directly to you unless otherwise specified, and you can use them to pay your bills as you see fit. • Works alongside your Health Savings Account (HSA) • Coverage is available for you, your spouse and eligible dependent children.* • You don’t need to answer medical questions or have a physical exam to get accident coverage.** • Benefit payments aren’t reduced by any other insurance that you may have with another company. Give your benefits a boost We know that more complicated or severe accidents result in more expensive medical bills and more disruption in your life. Group Accident includes a Benefit Booster*** to provide additional financial support for serious accidents. If you have more than $5,000 in payable benefits for a covered accident, we will give you a $500 boost to your benefits to help you with whatever expenses you have. Flexible coverage Your plan provides coverage for injuries and services from everyday mishaps to catastrophic events, including injury, treatment and recovery care benefits. PRE-EXISTING CONDITION LIMITATION We will not pay benefits for Hospital Confinement or any of the following benefit(s) for any covered person when such loss results from a pre- existing condition as defined in this certificate, unless the covered person has satisfied the pre-existing condition limitation period shown on the Certificate Schedule: - Rehabilitation Unit Confinement - Specified Critical Illness - Daily Hospital Confinement - Outpatient Surgical Procedure - Inpatient Mental and Nervous - Diagnostic Procedure GENERAL EXCLUSIONS We will not pay any benefits for injuries received in accidents or for sicknesses which are caused by, contributed to by or occur as a result of the covered person’s: - Addiction to alcohol or drugs, except for drugs taken as prescribed by his physician. - Treatment for dental care or dental procedures, unless treatment is the result of a covered accident. - Undergoing elective procedures or cosmetic surgery. This includes procedures or hospital confinement for complications arising from elective or cosmetic surgery. This does not include congenital birth defects or anomalies of a child, or reconstructive surgery. - Committing or attempting to commit a felony, or engaging in an illegal occupation. - Having a disorder including but not limited to affective disorders, neurosis, anxiety, stress and adjustment reactions. Alzheimer’s Disease and other organic senile dementias are not considered mental or nervous disorders. This exclusion does not apply to the Inpatient Mental and Nervous benefit, if included. - Dependent child’s pregnancy, including services rendered to her child after birth. Complications of pregnancy will be covered to the same extent as any other covered sickness. - Committing or trying to commit suicide or his injuring himself intentionally, whether he is sane or not. - Involvement in any act of war, declared or undeclared, or serving in the armed forces of any country or authority. Losses as a result of acts of terrorism committed by individuals or groups will not be excluded from coverage unless the covered person who suffered the loss committed or contributed to the act of terrorism. HOSPITAL CONFINEMENT LIMITATIONS We will not pay benefits for hospital confinement or daily hospital confinement due to any covered person giving birth within the first nine (9) months after the coverage effective date of the certificate as a result of a normal pregnancy, including cesarean. Complications of pregnancy will be covered to the same extent as any other covered sickness. WELL BABY CARE LIMITATION We will not pay benefits for hospital confinement or daily hospital confinement, if included, of a newborn child following his birth unless he is injured or sick.
